Documents You May Need to Complete Your Sprintax Calculus Profile

Sprintax will list the documents you need to provide at the bottom of your Tax Summary Report under "Supporting Documents (Copies)." In addition to the documents listed on this page, there may also be additional documents depending on immigration status. See Required Documents by Immigration Status for common examples.

Commonly Needed Documents

The list below shows some commonly required immigration documents at Harvard. The specific documents you will need to provide depend on your individual situation.

Passport ID page and pages with stamps/ stickers If you didn’t get a stamp/sticker in your passport, you can provide a copy of your Form I-94, Arrival/Departure Record.
Copy of your Visa Not all international payees will have a visa. If you are on an ESTA/VWP, please provide a copy of your approval from the online system. Canadians should provide a copy of their passport and Form I-94.
Copy of your immigration documents (See detailed list of Required Documents by Immigration Status)

The exact number will depend on your immigration status. J Visa holders are issued DS-2019, and F visa holders are issued an I-20.

I-20, Certificate of Eligibility for Nonimmigrant Status: A designated school official (DSO) issues this form to an F or M nonimmigrant after the student receives acceptance to a Student and Exchange Visitor Program-certified school. A Form I-20 identifies the F or M nonimmigrant (student or dependent) and the DSO, as well as other important information, such as the school and the program of study.

Form DS-2019, Certificate of Eligibility for Exchange Visitor Status: This form is generated by the Department of Homeland Security’s system SEVIS (Student & Exchange Visitor Information System) and is provided by your sponsoring institution.
 

Form I-94, Arrival/Departure Record. Created electronically upon arrival to the Unted States. You can download a copy from the Department of Homeland Security (DHS) website or from the CBP One™ mobile app

U.S. Tax Forms Sprintax May Require You to Sign and Upload

Sprintax will automatically generate the U.S. tax forms you need to sign. Sprintax allows users to electronically sign their tax forms; per IRS requirements, Sprintax’s electronic signature function requires two-factor authentication using the Google or Microsoft Authenticator apps. Alternatively, users can print, sign, scan and upload signed documents into Sprintax’s Document Exchange.

Can also be used to claim tax treaty exemption for certain types of income, such as non-service fellowship income and royalties.
Form 8233, Exemption From Withholding on Compensation for Independent (and Certain Dependent) Personal Services of a Nonresident Alien Individual – 2023 Used by nonresident individuals eligible to claim tax treaty exemption on compensation for dependent or independent services 
Form W-4, Employee's Withholding Certificate Provides Harvard information needed to determine how much federal income tax to withhold from an employee’s pay. Nonresident aliens must follow specific instructions when completing this form
State Tax Form – Form M-4, Massachusetts Employee’s Withholding Exemption Certificate Provides Harvard information needed to determine how much Massachusetts state income tax to withhold from an employee’s pay.
